Product summary

The SIM incident analysis method was developed to meet the need for a SIMPLE method to investigate the causation of incidents with a potential of slight injury or damage only. The method does not pretend to present an extensive and detailed analysis of an incident; a SIM tree shows a picture of how the incident process developed and what influences made it possible to happen. To investigate high potential incidents organisations use more advanced investigation and analysis techniques (e.g. Tripod), but these tools are considered an overkill for the more simple incidents.

Features

SIM software offers a step by step approach that helps any user to set-up an incident analysis tree that shows:

  • The EVENT: this is the reason why the incident analysis was initiated. An ‘unplanned deviation from the expected’ has happened, and we want to know how that was possible
  • CAUSATION: HOW could that event happen? The method asks to indicate how the event could occur. Thereafter this question is repeated 4 times, so the tree will show a five (5) level causation path. The deepest level is called ‘Underlying Cause’
  • INTERVENTION: nodes (a unique feature the SIM tree). These nodes show what measures could have stopped the incident process, but in reality have not managed to do so
  • FAILURE: The reason why the intervention was not successful is also shown in the SIM tree

MULTIPLE CASUATION PATHS: Many times there is more than one argument HOW an event could evolve; therefore a SIM tree can show two different causation paths to identify ‘What was of influence too’.

  • A standardized incident analysis report is automatically generated by the SIM software, which can be further edited in MS-Word
